The Providence Fire Department is an all hazards emergency service organization that provides Fire, Emergency Medical Service, Rescue and Hazardous Material response to the people of Providence. We have been serving Providence as a professional department since 1854. We are extremely proud of our members and the service they provide. In order to improve our service we need your input. We would like to know from the public what we are doing well and how we can improve our service.
Your information will be shared with the member of the leadership of the department as well as neighborhood services. Therefore; this form should not include any confidential information. If you have a formal complaint you will be contacted by a member of the Command staff who will direct you on how to proceed with that process.
