The Department of Vital Statistics maintains records on births, deaths, and marriages within the City of Providence.
This department also provides certified copies of these records - for a fee - and is also responsible for the issuance of marriage licenses.

Instructions: For fastest service, please fill out and bring form to Providence City Hall, Office of the Registrar of Vital Statistics, First Floor, Room 104, 25 Dorrance Street, Providence, RI 02903.
Mailing Instructions: Fill out and mail with check to Providence City Hall, Office of the Registrar of Vital Statistics, First Floor, Room 104, 25 Dorrance Street, Providence, RI 02903. Please allow two weeks for processing by mail.

If you do not have a valid state id, and are seeking a vital record, please bring any TWO (or any one item twice) from the list below.
